Worcester nonprofit Seven Hills Family Services, an affiliate of the Seven Hills Foundation, has been selected by the Executive Office of Health and Humans Services to lead agencies across Central and Eastern Massachusetts in coordinating care for 4,500 Medicaid patients, the nonprofit said in a statement last week.

Doing business as the Massachusetts Care Coordination Network, Seven Hills and five behavioral health partner agencies will provide coordination for MassHealth patients with long-term care and support needs, Seven Hills said. MCCN will operate in the Central, Southeastern and Northeastern regions of Massachusetts.

Seven Hills was selected under the Long Term Services and Supports Community Partner program, a five-year program designed by the state in response to a national redesign to the federal Medicaid program.
